Before Surgery: Living with Daily Frustration
This patient came to the Gynecomastia Center of Boston after dealing with long-term chest fullness that didn’t match the rest of his body. He described his gynecomastia as one of the few things holding him back from feeling confident in everyday life. The tissue was firm and noticeable both in clothes and without a shirt.
It projected prominently from the side view and made it difficult for him to wear fitted clothing comfortably.
Even though he had worked to maintain a healthy weight, the fullness remained, a sign that glandular tissue, not just fat, was the underlying issue. He was ready for a solution that could finally help him move past this.

Week 0: Surgery Day
After an in-depth evaluation, we determined that liposuction-assisted gynecomastia surgery would provide the best outcome. This approach allowed us to:
- Remove fatty deposits with power-assisted liposuction
- Excise dense glandular tissue hidden beneath the areola
- Sculpt a flatter, more proportional chest
The procedure was performed under anesthesia for maximum comfort. Small, discreet incisions were made along the natural borders of the chest. The surgery lasted under two hours.
The patient returned home the same day, wearing a compression vest to aid healing and support the new contour.
Week 1: The First Look
At his one-week follow-up, early improvement was already visible. There was some swelling, as expected, but the chest was clearly flatter. The dramatic roundness that had previously pushed the nipple outward was gone.
The patient was already feeling more optimistic. He could see where the new shape was heading, even with compression and some post-surgical puffiness still present.
Week 3: Improved Shape and Smoother Contour
Three weeks post-op, the swelling had begun to subside significantly. The side view of the chest had changed the most. The protrusion was no longer there, and the transition from chest to abdomen was much more natural.
The patient remained diligent about wearing his compression garment and following all recovery instructions. We discussed the possibility that, in the future, he may wish to consider skin-tightening options. While his result looked excellent overall, some minor looseness remained.
This is common in patients with a thicker skin envelope or long-standing gynecomastia and does not take away from the success of the initial procedure.
Week 6: Return to Normal Activity
At six weeks, the patient had resumed light exercise and was cleared to return to full physical activity. He was back to wearing shirts that previously felt uncomfortable and mentioned how much easier it was to move and stretch without feeling restricted by his chest.
He continued to heal well, with the incisions fading nicely and no complications throughout recovery.

Outcome Summary
The photos tell the story best. From the front, the shape is significantly flatter and more balanced. From the side, the dramatic reduction in projection has restored a more masculine frame.
While the patient may eventually consider skin refinement for even greater definition, the core issue, excess gland and fat, was fully addressed.
This case highlights the power of combining precision liposuction with gland excision, especially when performed by a team that specializes in gynecomastia surgery. The patient now moves with more ease and carries himself with more confidence, and that, ultimately, is the goal.
